CIRCULAR
Amending and supplementing a number of articles of the Circular No. 66/2015/TT-BCA dated December 15, 2015, providing forms used in the grant and management of citizen’s identity cards, citizen identification archives, and the National Database on Population

Pursuant to the 2014 Law on Citizen Identification;
Pursuant to the Decree No. 01/2018/ND-CP dated August 6, 2018 defining the functions, tasks, powers and organizational structure of the Ministry of Public Security;
At the proposal of the Director of the Police Department for Administrative Management of Social Order;
The Minister of Public Security promulgates the Circular amending and supplementing a number of articles of the Circular No. 66/2015/TT-BCA dated December 15, 2015 of the Minister of Public Security, providing forms used in the grant and management of citizen’s identity cards, citizen identification archives and the National Database on Population.
Article 1. To amend and supplement a number of articles of the Circular No. 66/2015/TT-BCA dated December 15, 2015 providing forms used in the grant and management of citizen’s identity cards, citizen identification archives and the National Database on Population
1. Points m and n are added after Point 1, Clause 1, Article 3 as follows:
“m) Records of hand-over and receipt of dossiers for issuance, change and re-issuance of citizen’s identity cards (Form CC12);
n) Applications for certification of people’s identity card number (Form CC13)”.
2. Article 4 is amended and supplemented as follows:
“Article 4. Management of forms
1. The Ministry of Public Security shall uniformly manage the forms promulgated together with this Circular; print and distribute the CC11 forms to police agencies and local units.
2. The Police Department for Administrative Management of Social Order shall print, manage and develop forms CC01 and CC13 used at the National Center for Citizen Identification.
3. Police of provinces and municipalities shall print, manage and distribute forms CC01, CC02, CC08, CC12, CC13, DC01 and DC02 to the police agencies and local units under their management.
4. Forms CC02, CC03, CC04, CC05, CC06, CC07, CC09 and CC10 shall be printed directly from the computer when a police unit is carrying out procedures for issuing, changing and re-issuing a citizen’s identity card. The printed form shall contain the information extracted and summarized from the Citizen Identification Database within the scope of storage of each unit.
5. Forms CC01, CC02, CC04, CC05, CC06, CC07, CC08, CC09, CC10, CC12, CC13, DC01 and DC02 shall be printed on 210 mm x 297 mm (A4) paper. Form CC03 shall be printed on 148 mm x 210 mm (A5) paper, Form CC01 and Form CC08 shall be printed on both sides of the paper. Form CC11 shall be printed on 250gsm Kraft paper with the size of 250 mm x 330 mm.
6. The forms specified in this Circular shall printed with black ink on white paper. Form CC11 shall printed with black ink on yellow-brown paper. Upon printing, the content of the forms must not be changed; Police agencies and local units shall be responsible for managing the printing and distribution of forms and keeping track of them in books.”
3. Clauses 2 and 3, Article 5 are amended and supplemented as follows:
“2. The Police Department for Administrative Management of Social Order shall assume the prime responsibility for and coordinate with the Department of Planning and Finance and relevant units in making budget estimates for the printing and distribution and the paper supply for the printing of Forms CC01 and CC11, the paper supply for the printing of Form CC02 used at the National Center for Citizen Identification and Form CC11 distributed to police agencies and local units.
Every September, the police agencies and local units shall make estimates of their demand for Form CC11 in the following year and send them to the Police Department for Administrative Management of Social Order so as for it to report to the Ministry of Public Security.
3. The police agencies and local units shall make budget estimates for the printing and distribution of forms to be used in the work of issuing and managing identity cards, citizen identification archives and the National Database on Population specified in Clauses 3 and 4, Article 4 of this Circular.”
4. Clause 1, Article 7 is amended and supplemented as follows:
"1. Form CC01 shall be used by a citizen to declare information about his/her personal identity when requesting the issuance, change or re-issuance of his/her citizen’s identity card (even if the citizen declares on the online public service website).”
5. Point h, Clause 2, Article 7 is amended and supplemented as follows:
“h) Section “Place of birth registration”: Fill in the administrative name of the commune, district, and province stated in the citizen's birth certificate. If the birth certificate does not fully state the names of all communal-, district- and provincial-level administrative locations, the administrative name that is stated in such birth certificate shall be still filled in the section. If the administrative name is changed, the new administrative as prescribed by the law provisions shall be filled in the section;”
6. Point i, Clause 2, Article 7 is amended and supplemented as follows:
“i) Section “Hometown”: Fill in the administrative name of the commune, district, and province stated in the birth certificate and household registration book. If those papers do not fully state the names of all communal-, district- and provincial-level administrative locations, the administrative name that is stated in such papers shall be still filled in the section. If the administrative name is changed, the new administrative as prescribed by the law provisions shall be filled in the section;”
7. Point o Clause 2 Article 7 is amended and supplemented as follows:
“o) Requests of the citizen:
- Section "Issuing, changing or re-issuing citizen’s identity cards": Fill in “Issue new citizen’s identity card” in case of first-time issuance; Fill in “Change of citizen’s identity card” if the citizen’s identity card is damaged or expired, the card malfunctions, the information thereof is changed, or the citizen requests to change the card, write the issue and change; Fill in “Re-issue citizen’s identity card” if the card is lost or the citizen’s Vietnamese nationality is restored;
- Section “Delivering the citizen’s identity card to the citizen's address”: If a citizen requesting the issuance, change or re-issuance of his/her citizen’s identity card wants the card to be delivered to his/her address, fill in "Yes" as well as the address of receipt and the contact phone number; if he/she does not want so, fill in “No”;
8. Point q is added after point p, Clause 2, Article 7 as follows:
“q) The two-dimensional bar code encodes all personal information of the citizen and the information for management on the online Citizen Identification Declaration.
Section "Appointment time": Appoint a specific date for the citizen to carry out procedures for issuing, changing and re-issuing his/her citizen’s identity card at the citizen identification management agency; Section "At…": Fill in the name of the citizen identification management agency that the citizen registers to carry out the procedures for issuing, changing and re-issuing his/her citizen’s identity card”.
9. Clause 3, Article 8 is amended and supplemented as follows:
“3. Number of the identity card: The officer who handles the application dossier for issuance, change or re-issuance of the citizen’s identity card, upon acknowledging the issuance, change, or re-issuance is approved (receiving the citizen’s identity card or basing on the data of the citizen’s identity card approval updated on the system), shall write the number of such citizen’s identity card in the form for collecting identity card information (Form CC02)".
10. Clause 1, Article 12 is amended and supplemented as follows;
"1. Form CC06 shall be filled in by the National Center for Citizen Identification in order to propose the leadership of the Police Department for Administrative Management of Social Order to approve the application dossiers for issuance, changing or re-issuance of identity cards. This form shall be based on the list of application dossiers for issuance, changing or re-issuance of citizen’s identity cards sent by the police departments of provinces and municipalities.”
11. Clause 1, Article 14 is amended and supplemented as follows:
"1. Form CC08 shall be used by the provincial police and district police to make statistics on results of the issue and management of citizen identification. On a monthly, quarterly, biannual, or annual basis, the provincial-level police departments shall send reports to the Police Department for Administrative Management of Social Order.”
12. Article 17a is added after Article 17 as follows:
“Article 17a. Records of hand-over and receipt of dossiers for issuance, change and re-issuance of citizen’s identity cards (CC12)
1. Form CC12 shall be used by units carrying out procedures for issuance, exchange and re-issuance of citizen’s identity cards to hand over application dossiers of issuance, exchange and re-issue of citizen’s identity cards to the units for management of citizen identification archives under the Police Divisions for Administrative Management of Social Order under the Police Departments of the provinces and municipalities. The units in charge of receiving the application dossiers shall check and fully receive all the dossiers as prescribed.
2. How to record information:
a) Section “on…. hour, day…, month…, year…”: Fill in the time and date of handing over the dossiers; Section “at…”: Fill in the location where the dossiers are handed over;
b) Sections "Representative of the handing-over unit" and "Representative of the receiving unit ": Fill in the full names, ranks, positions, and working units of the officers representing the handing-over unit and the receiving unit;
c) Section "Total number of dossiers": Fill in the total number of dossiers to be handed over, including the "Total number of dossiers for issuance of new cards", "Total number of dossiers for change of cards", "Total number of dossiers for re-issuance of cards".
d) Section "A list of dossiers from No… to No... is attached herewith": Fill in the ordinal numbers of the dossiers based on the list of dossiers to be handed over.
3. Section “The handing-over unit” and “The receiving unit”:
a) In case of handing over dossiers between units under the Police Division for Administrative Management of Social Order, the team leaders of the unit shall sign to certify the hand-over record. Officers representing the handing-over unit and the receiving unit shall sign and clearly state their full names in the receipt record.
b) In case of handing over dossiers between the district-level police and the Police Division for Administrative Management of Social Order, the leaders of the two units shall sign and stamp their units’ seals in the hand-over record. For dossiers issued at the National Center for Citizen Identification under the Police Department for Administrative Management of Social Order, the leaders of the center and the Police Division for Administrative Management of Social Order which receives the dossiers shall sign the hand-over record. Officers representing the handing-over unit and the receiving unit shall sign and clearly state their full names in the receipt record.”
13. Article 17b is added after Article 17a as follows:
“Article 17b. Applications for certification of people’s identity card number (CC13)
1. Form CC13 shall be used in cases where a citizen has been issued with a citizen’s identity card but has not yet been issued a certificate of his/her people’s identity card number or has lost his/her certificate of people’s identity card number.
2. How to record information:
a) Section “Last, middle and first names”: Fill in the full family name, middle name and first name of the citizen stated in his/her birth certificate in full capital letters;
b) Section “Date of birth”: Enter the full date of birth of the citizen who is issued, changed or re-issued the identity card. The day of birth shall be written with 02 digits; The year of birth shall be written with four digits. The month of birth from March to September shall be written with 01 digit and the remaining months shall be written with 02 digits;
c) Section “Gender”: Fill in “Male” if the citizen is male, “Female” if the citizen is female;
d) Section “Ethnicity”: Fill in the ethnicity of the citizen applying for a certificate of people’s identity card number, which is stated in his/her birth certificate or papers issued by a competent agency proving his/her ethnicity;
dd) Section "Nationality": Fill in the nationality of the citizen applying for a certificate of people’s identity card number, which is stated in his/her birth certificate or certificate of Vietnamese nationality issued by a competent agency;
e) Section "Place of birth registration" and Section "Hometown": Fill in the information as prescribed in Points h and i, clause 2, Article 7 of this Circular.
g) Section “Place of permanent residence”: Fill in fully and accurately the information as stated in the household registration book.”
14. The forms issued together with Circular No. 66/2015/TT-BCA are replaced as follows:
a) The citizen identification declaration (Form No. CC01) issued together with Circular No. 66/2015/TT-BCA shall be replaced with the citizen identification declaration (Form No. CC01) issued together with this Circular.
b) The proposal for approval of application dossiers for issuance, change and re-issuance of citizen’s identity cards (Form No. CC06) issued together with Circular No. 66/2015/TT-BCA shall be replaced by the proposal for approval of application dossiers for issuance, change and re-issuance of citizen’s identity cards (Form No. CC06) issued together with this Circular.
c) The certificate of people’s identity card number (Form No. CC07) issued together with Circular No. 66/2015/TT-BCA shall be replaced by the certificate of people’s identity card number (Form No. CC07) issued together with this Circular.
d) The form for adjustment of citizen identification information (Form No. CC09) issued together with Circular No. 66/2015/TT-BCA shall be replaced by the form for adjustment of citizen identification information (Form No. CC09) issued together with this Circular.
e) The form for collection of residential information (Form No. DC01) issued together with Circular No. 66/2015/TT-BCA shall be replaced with the form for collection of residential information (Form No. DC01) issued together with this Circular.
15. Article 21 is amended and supplemented as follows:
“Article 21. Responsibilities for implementation
1. Director of the Police Department for Administrative Management of Social Order shall guide, inspect and urge the implementation of this Circular.
2. Heads of agencies directly under the Ministry of Public Security, Directors of Police Departments of provinces and municipalities, relevant agencies, organizations and individuals shall be responsible for implementing of this Circular.
In the course of implementation, if there are difficulties or problems arising, the police agencies and local units shall report them to the Ministry of Public Security (via the Police Department for Administrative Management of Social Order) for timely guidance.”
Article 2. Effect
This Circular takes effect from November 18, 2019.
Article 3. Responsibilities for implementation
1. The director of the Police Department for Administrative Management of Social Order shall guide, inspect and urge the implementation of this Circular.
2. Heads of agencies under the Ministry of Public Security, Directors of Police Department of provinces and municipalities, relevant agencies, organizations and individuals shall be responsible for implementing this Circular.
In the course of implementation, if there are difficulties or problems arising, the police agencies and local units shall report them to the Ministry of Public Security (via the Police Department for Administrative Management of Social Order) for timely guidance./.
